Medulloblastoma with leptomeningeal metastases.

چکیده
A 27-month-old previously healthy boy was brought to the hospital presenting with signs of raised intracranial pressure including vomiting, irritability, adducted right eye, and fever. Urgent magnetic resonance imaging (MRI) of the brain showed a small T1and T2-weighted isointense nodule (Fig 1), which enhanced avidly over the right lateral aspect of the fourth ventricle after intravenous contrast (Fig 2). Features on diffusion-weighted image (DWI) of bright signal intensity and apparent diffusion coefficient (ADC) map of low values (0.67-0.78 x 103 mm2/s) in the abnormal area suggested restricted diffusion of the nodule at the fourth ventricle (Fig 3). In addition, diffuse nodular supratentorial and infratentorial leptomeningeal enhancement was noted (Fig 4). Considering the age of the patient, and the location and characteristic imaging features of the lesion, a diagnosis of medulloblastoma with leptomeningeal metastases was proposed by the radiologists. The diagnosis was later confirmed by excisional biopsy.

Extraneural metastases in medulloblastoma.
Medulloblastoma is the most common childhood malignant tumor of central nervous system, but it may also occur in adults. It presents high invasive growth with spreading of tumor cells into the leptomeningeal space along the neuroaxis early in the course of the disease. Extraneural metastases are rare but frequently lethal, occurring only in 1 to 5% of patients, and are related, in the most of c...

متن کاملLoculated intracranial leptomeningeal metastases: CT and MR characteristics.
Studies of twenty-five patients with loculated leptomeningeal tumor metastases diagnosed by CT and/or MR were analyzed retrospectively. Medulloblastoma was the most frequent primary tumor (8/25, 32%). Four subgroups of loculated patterns were identified. Type A included mass(es) limited to the subarachnoid space without obvious direct parenchymal infiltration; this pattern occurred in 12 patien...

Evidence about distribution patterns of brain metastases with regard to breast cancer subtypes and its influence on the prognosis of patients is insufficient. Clinical data, cranial computed tomography (CT) and magnetic resonance imaging (MRI) scans of 300 breast cancer patients with brain metastases (BMs) were collected retrospectively in four centers participating in the Brain Metastases in B...
متن کاملA rare presentation of medulloblastoma in adults as primary leptomeningeal involvement
Medulloblastoma is one of the most common primary tumors of the central nervous system of children but it is uncommon in adult age. We report a case of young male presented with a history of generalized headache that was found to have medulloblastoma with diffuse leptomeningeal involvement without prominent mass lesion. To our knowledge, our patient is the second reported case of medulloblastom...
